O40: PRE-OPERATIVE MANAGEMENT AND SENSITISATION IN VASCULARIZED COMPOSITE ALLOTRANSPLANTATION: A SYSTEMATIC REVIEW

چکیده
Abstract Introduction Vascularised composite allotransplantation (VCA) permits like-for-like reconstruction following extensive soft tissue injuries. The initial management of injury can lead to the development anti-HLA antibodies through injury, transfusion and cadaveric grafting. role antibody-mediated rejection, donor-specific graft rejection in context VCA remains unclear. This systematic review aimed determine whether pre-operative strategies influence immunological outcome VCA. Method A MEDLINE, EMBASE CINAHL using a PRISMA-compliant methodology up February 2019 was conducted. Pre-operative, procedural long-term data were collected recorded for all recipients on an individual patient basis. Result search revealed 3,847 records which 114 met inclusion criteria reported clinical related 100 patients who underwent 129 transplants. Trauma (50%) burns (15%) most frequent indications Of studies, only one acute resuscitative management. Fifteen (14.7%) sensitized prior reconstructive transplantation with 80% incidence first post-operative year. Seven demonstrated vasculopathy, whom had panel reactive antibodies. Conclusion Currently employed predispose antibodies, adding already complex challenge Early appropriate care is warranted loss where needs should be balanced against mitigate burden, particularly as becomes feasible option future coverage. Take-home message Acute may sensitise potential transplant major injury. warranted.

Facial Vascularized Composite Tissue Allotransplantation
Reconstruction of severe composite defects of the face is challenging for reconstructive surgeons. Despite several reconstructive techniques, the functional and aesthetic results of these patients are not acceptable. As a major organ, face, has an utmost importance in daily life. The specific characteristics of face require not a reconstruction, but a replace with same tissue. With the evolutio...
متن کاملSplit Rejection in Vascularized Composite Allotransplantation
INTRODUCTION Graft monitoring following vascularized composite allotransplantation (VCA) relies primarily on serial skin biopsy. However, given that VCA comprised multiple tissue types, skin biopsy may not accurately reflect rejection in other transplanted tissue. METHODS A review of the literature on episodes of both acute and chronic rejection following VCA was completed. Special attention ...
متن کاملMouse Models of Experimental Vascularized Composite Allotransplantation
The clinical practice of vascularized composite allotransplantation (VCA) has been limited by the potential side effects of chronic immunosuppression. Studies using rodent models have been useful for dissecting mechanisms underlying immunological events induced by VCA and for developing protocols such as mixed chimerism for tolerance induction. Mouse models of VCA have great advantages over rat...
متن کاملCurrent status of vascularized composite tissue allotransplantation
Vascularized composite tissue allotransplantation (VCA) offers treatment options of complex functional deficiencies that cannot be repaired with conventional reconstructive methods. VCAs consist of blocks of functional units comprising different tissue types such as skin, bone, muscle, nerves, blood vessels, tendons, ligaments and others, and are thus substantially different from the compositio...
متن کاملMesenchymal Stem Cells as Immunomodulators in a Vascularized Composite Allotransplantation
Vascularized composite allotransplantations (VCAs) are not routinely performed for tissue reconstruction because of the potentially harmful adverse effects associated with lifelong administration of immunosuppressive agents. Researchers have been eagerly seeking alternative methods that circumvent the long-term use of immunosuppressants. Mesenchymal stem cells (MSCs) show promise as an immunomo...
